Maternity leave

It is equal to 70 days (in the case of multiple pregnancies - 84) before childbirth and 70 (with complicated childbirth - 86, with the birth of two or more children - 110) - after.

Holiday to care for the child

It is provided after maternity leave and can last until the child reaches three years of age.

How to write a statement

There are no strict requirements for the form. Perhaps your company has a standard version of the document that the HR department wants to see. Then it is better to ask them for a sample. But in general, the main thing is that the application contains all the necessary information.

Statement cap

For all types of vacations, it is drawn up in the same way. The heading - that is, the part that is usually located in the upper right corner, indicates the position of the addressee, the name of the company, his name, patronymic and surname, as well as from whom the application is being submitted.

We write data about the addressee in the dative case (to whom?), About ourselves - in the genitive (from whom?). In this case, the preposition "from" can be put or omitted, both approaches will be correct.

Title of the document

The word "statement" can be written in three ways:

Annual paid vacation

There are several options. If you just ask for a vacation, the entry will be very concise.

Everything is clear here: after the preposition "from" the date of the first day of the vacation is indicated, then the number of its days, which are counted from the specified date.

If the company has approved a vacation schedule - and this should happen annually - the application need not be written. Study leave

For paid leave, the following statement is suitable:

I would like to ask you to provide study leave with the preservation of the average salary for the period from December 20, 2021 to January 30, 2022, with a duration of 40 calendar days for passing the intermediate certification at the university.

I attach a call-out certificate from an educational institution.

Please note: the name of the educational institution must be indicated in the application. And the vacation period is not determined from a bunch of flounders, the period must coincide with the number of days indicated in the certificate-call from the university.

For study leave at your own expense, the wording needs to be slightly changed.

I would like to ask you to provide study leave without keeping the average salary for the period from July 20, 2021 to August 4, 2021, with a duration of 15 calendar days for passing entrance examinations at the university.

I am attaching a call-out certificate from an educational institution.

Maternity leave

The statement is also very succinct.

I would like to ask you to grant me maternity leave of 140 calendar days from September 1, 2021 and to pay a lump sum. The basis is a certificate of incapacity for work dated August 15, 2021 No. 101 010 202 020.

Holiday to care for the child

Again, you won't have to write a lot. You need to indicate why you are taking vacation and for what period.

When to apply

Usually the document is submitted two weeks in advance. There is no such requirement in the law, it is the employer who must notify the employee about the annual leave within this period. But, if possible, it is better to submit the paper in advance. This will give the company time to pay all that is due and decide how to organize work during your absence.
