The life hacker figured out what the erythrocyte sedimentation rate is connected with and how it is determined.
What is ESR
ESR Sed rate (erythrocyte sedimentation rate), or erythrocyte sedimentation rate, is one of the components of a general blood test, usually assessed by doctors along with other indicators.
For the first time, the ability of cellular elements to sink to the bottom of a test tube was noticed by a Polish doctor in 1897 by methods of measuring ESR. He called it an erythrocyte sedimentation reaction, which is why some older doctors continue to use the abbreviation ROE. But the connection of this analysis with various pathological conditions was established only in 1918 by the Swedish scientists Faohraeus and Westergren. They have developed a universal diagnostic method that is used all over the world - the Westergren method.
The researchers found that red blood cells are the most difficult nursing in blood cell therapy due to the fact that they have hemoglobin and iron atoms. Therefore, in a special solution, erythrocytes settle under the influence of gravity at a certain speed. But due to some components in the blood, this can happen faster.
ESR cannot accurately determine the disease, since the indicator often increases due to natural causes in healthy people.
Therefore, doctors assess the erythrocyte sedimentation rate only in conjunction with other abnormalities.
What does ESR depend on?
The membrane of all erythrocytes has a negative Clinical assessment of the results of laboratory tests - Nazarenko G. I., Kishkun A. A. electric charge, therefore, according to the laws of physics, cells repel each other. Various substances can change polarity, which are fixed on the surface of blood cells and cause them to stick together. For example, these are proteins involved in inflammatory reactions:
- immunoglobulins;
- cytokines;
- fibrinogen.
Sometimes ESR is accelerated by the Clinical assessment of the results of laboratory tests - Nazarenko G. I., Kishkun A. A. and other changes in the body. These include acid-base balance, shifted towards alcolosis, or alkalization, blood viscosity and a large amount of lipids, ionic charge of plasma. Studies also show that high rates of erythrocyte sedimentation rate appear when the shape of these blood cells changes, their number or size decreases, and hemoglobin levels decrease.
How ESR is determined
The analysis is carried out in different ways. Methods for measuring ESR, which differ in many parameters, including the indicators of the norm. It:
- Westergren's method. For him, you need 2 ml of blood from a vein, which is collected in a test tube, mixed with a reagent that protects against clotting. In this case, the resulting mixture, if necessary, can be stored in the laboratory refrigerator for up to 6 hours. Then the available blood is filled into a glass tube 200 mm long and 12.5 mm in diameter and left in an upright position for 1 hour. Later, according to the applied scale, they measure how many millimeters the sediment has descended down. Sometimes the research is carried out within 2 hours.
- Panchenkov's method ESR measurement methods. This is a modification of the previous method, which is used in the countries of the former USSR. It only requires 0.2 ml of finger blood, which cannot be stored. Therefore, the analysis is immediately recruited into a glass tube - a capillary, 100 mm long and a lumen diameter of less than 1 mm. The vessel is preliminarily washed with a solution that protects the blood from clotting. The capillary is fixed on a special stand in a vertical position, and an hour later the distance from the edge of the liquid to the border with the sediment is measured.
Despite the convenience of finger-stick blood analysis, this method of obtaining the material can adversely affect the ESR measurement methods on the result. If a laboratory assistant strongly massages or squeezes the pad of a finger, blood vessels expand in the tissues, inflammatory proteins are released, which can increase ESR.
What is the rate of ESR in the blood
Only a doctor can decipher the results of the study, you should not engage in self-diagnosis.
Laboratories can use different techniques. For the Westergren analysis, a good Sed rate (erythrocyte sedimentation rate) of ESR is 0-22 mm / h for men and 0-29 mm / h for women.
When conducting an examination according to the Panchenkov method, the following results are considered normal. Clinical assessment of the results of laboratory tests - Nazarenko G. I., Kishkun A. A.:
- newborns in the first weeks of life - 0-2 mm / h;
- infants up to 6 months - 2-17 mm / h;
- women under 60 years old - up to 12 mm / h;
- women after 60 years - up to 20 mm / h;
- men under 60 years old - up to 8 mm / h;
- men after 60 years - up to 15 mm / h.
